Add a page for how to attribute the Toolkit (in source and apps)

We see a lot of projects that re-purpose code from the Toolkit. We should have a proper guide on how to properly attribute our project.

I'll see if this is something the .NET Foundation itself can help with, I don't see a lot of great guidance on how to attribute open source within other projects vs. third-party notices.
