Create a workflow to file an issue upon failure of a specified workflow

I also have a free PagerDuty account, so I should see about having this also optionally page me (and whoever else wants to get paged, if they so choose). Hosting it here means it could be reused and whatever commit history here won't pollute the main repo.

That way, I won't miss publish errors so easily. It's happened more than once, and I'd rather know immediately that it failed rather than have someone complain hours later, long after I've stepped away.
