[Feature request] Allow add custom header to HTTP(S) upstream

Currently, Caddy forward proxy supports upstream, but it does not pass any information except the request address. If it supports settin custom HTTP headers for upstream, upstream can implement functions similar to user authentication based on source address and authentication information.
