Remove "colbyfayock" references

We appreciate Colby's significant contributions to these repos, but as he's no longer a core maintainer, we need to edit the references to his handle throughout the codebase. In the copyright line, for example, change his name to the company name. Watch for ties to his account and edit them to refer to Cloudinary.
