Consider whether to drop custom index support for profile access management

In bedrock-web-profile-manager, there is some code that would allow additional indexes to be added to user documents in the user EDV. We might want to remove that feature since it isn't very compositional in nature and produces problems, such as, ensuring those indexes stay up-to-date when refreshing zcaps stored in user documents.
