Add inline diff annotations

Now that we've got a framework for inline annotations, I'd like to add inline diff annotation support:

```php
// [code! add]
// [code! remove]

// [code! ++]
// [code! --]
```

These annotations should still support all of the regular ranges the others support, in addition to replacing the line number with a `+` or `-` character to signify the diff type.
