I'd suggest putting in the contact information for the evaluator, as we've done here for OpenACR

Ultimately, I think you want the names of the person (and/or organization) that asked for WCAG-EM to be generated and the name of the person (and/or firm) doing it.

There may be a need to ask for clarifications or to have something re-tested.
